If you don't want a custom form, you have to clear out the custom form
setting from the last one you used. To do that, go through the "New
Form" process, which will bring up a form, that is *derived* from your
custom form.
In the menu (with the form designer open):
File
Set Custom Form Class
Click the button that says "Clear Custom Form Class".
Click "OK"
Close the form in the form designer without saving it.
Start a new form, it should not be based on a custom form.
